Mechanical dyssynchrony indices from an electrophysiological contact mapping system predict response to CRT

چکیده
We present a methodology to extract mechanical dyssynchrony indices from an electrophysiological (EP) contact mapping system that was applied to a database of ten patients undergoing Cardiac Resynchronization Therapy (CRT), before and after the therapy. The mechanical dyssynchrony indices were obtained after post-processing high temporal resolution data recorded at several endocardial positions with the measuring catheter of the EP system. Velocity profiles of these catheter trajectories were analyzed to identify abnormal patterns of motion such as septal flash, whose presence is strongly related to a positive response to CRT in patients with left bundle branch block. Septal flash motion was identified in four cases, which was qualitatively validated by visual inspection of imaging data, and a strong correlation with response to CRT was observed a posteriori. Finally, the effect of biventricular pacing was clearly observable by a substantial reduction of the septal flash

BACKGROUND The power of echocardiographic dyssynchrony indices to predict response to cardiac resynchronization therapy (CRT) appears to vary between indices and between studies. We investigated whether the variability of predictive power between the dyssynchrony indices can be explained by differences in their operational definitions. METHODS AND RESULTS In 132 CRT-candidates (left ventricul...

BACKGROUND Whether mechanical dyssynchrony indices predict reverse remodeling (RR) or clinical response to cardiac resynchronization therapy (CRT) remains controversial. This prospective study evaluated whether echocardiographic dyssynchrony indices predict RR or clinical response after CRT. METHODS AND RESULTS Of 184 patients with heart failure with anticipated CRT who were prospectively enr...

BACKGROUND Mechanical dyssynchrony is a potential means to predict response to cardiac resynchronization therapy (CRT). We hypothesized that novel echocardiographic image speckle tracking can quantify dyssynchrony and predict response to CRT. METHODS AND RESULTS Seventy-four subjects were studied: 64 heart failure patients undergoing CRT (aged 64+/-12 years, ejection fraction 26+/-6%, QRS dur...
متن کاملStrain dyssynchrony index determined by three-dimensional speckle area tracking can predict response to cardiac resynchronization therapy
BACKGROUND We have previously reported strain dyssynchrony index assessed by two-dimensional speckle tracking strain, and a marker of both dyssynchrony and residual myocardial contractility, can predict response to cardiac resynchronization therapy (CRT). A newly developed three-dimensional (3-D) speckle tracking system can quantify endocardial area change ratio (area strain), which coupled wit...
متن کاملErythropoietin Levels in Cardiac Resynchronization Patients
Cardiac resynchronization therapy (CRT) is indicated in patients with advanced heart failure secondary to severe systolic impairment and refractory symptoms despite optimized medical treatment and evidence of electromechanical dyssynchrony with a QRS complex greater than 120 milliseconds (msec). Approximately 20%-30% of patients who receive CRT fail to respond with little improvement in subject...
